Case Management
Neumann Family Services' Case Management Program is dedicated to helping individuals maintain independence in the community by addressing their physical, financial, educational, recreational, environmental and mental health needs. Case mangers work in conjunction with the individuals and their caregivers to insure thorough and effective coordination of care. The case management program provides a comprehensive array of support services including:
- Providing assistance in receiving medical, dental, podiatry and vision services, as well as lab work and preventative health screenings
- Coordination of state and federal government entitlements (Medicaid, food stamps, cash assistance, Medicare, SSI, SSA)
- Linkage to GED classes and other educational/vocational training programs
- Accessing recreational resources in the community
- Collaborative community advocacy and representation
- Coordination of appropriate living arrangements and/or assistance in obtaining safe and affordable living arrangements
- Crisis response services
- Educational and supportive services to families
